The Sherbourne Health Centre is an urban primary health care centre serving the diverse communities of southeast Toronto since 2003.
We seek to reduce barriers to care by taking a comprehensive approach to primary health care – an approach grounded in the understanding that to truly build healthy communities, services must extend beyond basic health care.
Our interdisciplinary team includes physicians, nurses, a nurse practitioner, counselors, medical secretaries, health promoters, and outreach workers – who are all dedicated to delivering high quality programs and services that honour your unique health needs and particular circumstances.
